Print this registration form and complete it. Pay special attention to the Emergency Contact Information and Pick Up Authorizations sections. For your child’s safety, if a person’s name is not on the list, s/he cannot pick up your child.
Mail the completed registration form with full payment to Saint John Bosco Parish, 16 Washington Street, Port Chester NY 10573

Drop off Registration forms at Religious Education Office 135 South Regent Street (Corpus Christi Gymnasium Entrance).
